It's difficult to help when you omit so many important details... like......

1: Used in a desktop or laptop pc?

2: Motherboard/computer brand and model?

3: Is the computer custom made or factory made?

Conexant doesn't make PCI modem cards, they make chipsets so the correct interface is "integrated on the motherboard".
And the correct chipset number is CX11252-11.
