MODEM is not a valid model number and Lucent doesn't make modems, only chipsets. Modems also do not have a SCSI interface. Most internal modems have PCI/ISA/PCI-E interface. External modems usually come with USB or serial port interface.
If this device came pre-installed in your pc at the time or purchase, we will need to know what the pc brand and model is.
If it's a retail (add-on) product, we will need to know who made it and what Lucent chip it has.
