Damn ISA modem is ancient! It acctually has the dip switches for choosing the com port! My guess is it's a 14.4 or 28.8 baud modem.

It's a 2400 baud on modem and 9600 baud on fax. Toss it and get on ebay and buy yourself a good 56k modem for a few bucks.
